I venture to say it's both Dietz and Blair. Dietz on the left-hand side; Blair in the middle and on the right-hand side. Blair said she did wear the Dick Smith vomiting apparatus in one of the behind-the-scenes clips or in an interview … just don't remember which one. If it is only Dietz, the makeup looks more like Blair than the actual slow vomiting scene that Dietz performed in the film.

I think Linda only tested Dick Smith's apparatus once or twice, it never went any further than that. She hated the whole thing. Not sure if there's even any test footage with her, it mostly all looks like Eileen.

Sof is right, folks. I couldn't remember where I saw Blair in the Dick Smith vomiting apparatus, but Movie Magic rang a bell and I own it on VHS (yes, that really old dead technology). I haven't seen it in some time because I don't own a VHS player anymore.
At any rate, you can see in the video below without argument that Blair is indeed at 1:51. The one thing that always stood out to me about Dietz, despite Dick Smith's clever makeup, is that Dietz's nose is so much narrower than Blair's pug nose. They never matched which I found somewhat odd because Smith could have easily molded Blair's nose and applied the same demon makeup to Dietz's face. The appliances were made of foam rubber and glued on with mineral spirits gum. Maybe I'm missing something.
You will see Dietz in the vomiting apparatus at 2:16. I think we may be hung on the fact that Dietz did perform this for the filmed version. However, wouldn't it make sense for Friedkin to want to do test footage with Blair just as well as Dietz? You'll notice that the demonic makeup had not been perfected yet looking at Blair's eyebrows and I can only imagine that lots of test footage was necessary. Yes, I realize that this topic is about the BTS material and I can't remember if test footage and BTS material was blended into one documentary or just came about through various sources.
Oh, and please … let's not let this end up in a trivial pissing contest. I put a lot of work in this post a very long time ago and posted a series of photos that are no longer available (probably removed from the original posting site that is now defunct). I can honestly say that I can see both Dietz and Blair. If you see only Blair, go for it. If you see only Dietz, go for it. If you see both Blair and Dietz, go for it. That's the wonder of movie makeup and special effects … we may never know true answers … just as it was intended.
